Aspergillus terreus Peritonitis in a Child on Continuous Ambulatory Peritoneal Dialysis: A Case Report from Pakistan

Background: Aspergillus peritonitis is a rare, potentially fatal complication of continuous ambulatory peritoneal dialysis (CAPD). Several cases fungal (FP) caused by species have been reported worldwide; however, there paucity data from Pakistan. Here, case congenital nephrotic syndrome in patient who progressed to Stage 5 chronic kidney disease when she was 8 years old, which managed CAPD and other supportive therapies. The developed FP later succumbed death despite appropriate management. Early diagnosis careful interpretation culture results are, therefore, important for the treatment these patients. Case presentation: outlined this report had syndrome, disease, on CAPD. She presented with peritonitis. Initially, broad spectrum antibiotics were started, samples did not yield any growth. Later, consecutive taken grew terreus, septate hyphae seen direct smear sample. Voriconazole started immediately, patients’ catheter removed. However, patient’s condition deteriorated and, after prolonged intensive care high ventilator support, expired. Conclusion: A. terreus an uncommon deadly pathogen that does respond commonly used antifungal treatments such as amphotericin B. CAPD-related requires early use newer drugs voriconazole or caspofungin.
